‘My Eyes Were So Puffy I Couldn’t Even Open Them’

Amanda Preisinger is nervous about her child’s upcoming 13th birthday party. It’s not due to the usual reasons associated with a house full of clamorous adolescent children, but because it’s the initial occasion she will showcase her recently altered face to friends and relatives. “Clearly I’m going to inform all guests as they arrive, ‘Just so you know, this is not the way I appear,’” says the 30-year-old real estate agent from Southern Florida.

Her appearance is, well, a little startling – her face puffed up and preternaturally lifted, as though secured by heavy-duty tape. Her altered – and she’s keen to stress, short-term – look is the result of six cosmetic procedures, including an endoscopic mid-facelift, conducted by a surgeon in Turkey’s Istanbul, last month. “My spouse teared up when he viewed me for the first time because I wasn’t able to even open my eyes. That indicates puffy I was,” she tells me via online call from her home. “I needed to inform him: ‘Honey, I’m okay, I’m not hurting. I just look like someone attacked me.’”

Increasing Popularity of Youthful Facial Lifts

According to plastic surgeons, Preisinger is among a rising count of individuals choosing to have a rhytidectomy in their 20s and 30s – significantly before a ten years before most doctors’ usual candidate age of forty to sixty. (Though most surgeons are eager to highlight the industry edict of: “We address genetics, not years.”) “I have individuals in their late twenties requesting facial lifts,” states based in London aesthetic plastic surgeon Georgios Orfaniotis, a ex-NHS doctor who focuses on the head and neck. “It’s a major surgery, and I’m a somewhat worried when I see individuals opting for it as a personal preference.”

Explaining the Facelift Procedure

A facelift, also known as a rhytidectomy, lifts the tissues in the face that begin to droop as we age. “Human faces are built a little like layers of an onion,” says based in Kent facial plastic surgeon Marc Pacifico. “Skin and fat on the surface, then a stratum of supportive tissue called the superficial musculoaponeurotic system. Think of the SMAS as the structural foundation of the face. And that’s what causes aging; that’s what becomes lax and drags the dermis down with it.”

You endanger operating on individuals that have underlying problems. It’s not a practice for an ethical plastic surgeon to follow

Previously reserved of moneyed older people, “filler fatigue” – when excessive use of dermal fillers expands the face and leads to looseness in the skin – alongside the widespread use of weight-loss drugs, low-cost medical tourism, and the development of new, famous-figure-promoted surgical techniques are driving a different type of customer towards this major operation. Reports indicate an eight percent rise in facelifts over the past 12 months in the United Kingdom; while a study revealed that the percentage of youthful candidates is growing, with one-third of procedures now performed on individuals aged 35-55.

“People are asking now to look like the optimal form of themselves and naturally the methods are evolving,” comments Orfaniotis. At present, the operation du jour is the deep plane facelift, a technique evangelised by figures including Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Advanced Lifting Method

While a conventional rhytidectomy involves lifting the superficial layer, the comprehensive lift loosens the underlying structures beneath it, enabling doctors to reshape the face by moving the underlying layers, and avoiding the tight, stretched look sometimes wrought by older methods. “We avoid placing tension on the skin because we’re targeting deeper, to the underlying structures,” says prestigious London cosmetic doctor Rajiv Grover, who focuses on the advanced method. “It is the deeper tissue that is repositioned. The skin follows along as a passenger.” Elevating the entire soft tissue layer as a single unit allows for a authentic-appearing and more durable outcome. It additionally implies that the surgery is not just being used for the express purpose of anti-ageing – or revitalization, as the cosmetic field describes it – but for “beautification” and facial restructuring, too. “We receive numerous requests from patients aged 28 to 35 for this purpose,” says Grover.

Individual Experience

Preisinger had not intended on getting a facelift – at minimum not at the age of 30. But years of using hyaluronic acid dermal fillers in an attempt at “facial balancing” implied that by the time she entered her late twenties, she was “damaged”. “I didn’t undergo this because I wanted to look young,” she says. “I underwent it because the injectable harmed my face. I wish I had never done it. If I didn’t have the injectable, I don’t believe I would be where I’m at currently.”

If injectable substances completely disappear has historically been a point of contention in the aesthetics industry. “Research have shown that they seldom fully dissipate,” states Grover, “but they migrate and can block drainage pathways. One of the contributors to what we term ‘puffy appearance’ is also the fact that in a sense, the face becomes slightly fluid-filled because its ability to drain lymphatic fluid has been reduced.” Although studies into the topic is preliminary, warnings on dermal fillers’ potential impact on the body’s drainage have been released, and further research initiated. An esteemed facial plastic surgeon, commenting anonymously, mentioned he would avoid using fillers in a client because of the dangers they pose. “Many surgeons avoid discussing this, because the manufacturers who make filler can be pretty aggressive.” He’s heard stories, he says, of plastic surgeons being landed with legal actions after expressing worries.

Decision and Procedure

For Preisinger, once she started using injectables, she believed she had to keep replenishing it – particularly when it started to migrate to other parts of her face. She was only twenty-eight when a nearby cosmetic doctor in the state suggested that a facial and neck surgery might be what she needed to exit the injectable hamster wheel. Two years later, she ended up choosing between a list of recommended hotels in Istanbul, texted to her by the clinic assistant at her surgeon’s office.

Initially, Preisinger had scheduled an upper and lower blepharoplasty – commonly called eyelid surgery – but her surgeon recommended that a facial lift was the correct option for the looseness she was finding in her face. She booked a brow lift, a cheek lift and a neck lift. The day before her surgery, 24 hours post she’d touched down solo in the country, she opted to include a mouth enhancement. “Then he said, ‘We’re going to lift the lower eyelid puffiness.’ He added, ‘I believe if we extract some cheek fat, it will sculpt your face.’ Thus I ultimately adding two more procedures,” she recalls. She spent $22,000 (£16,500) for the six-hour surgery, plus a three-day|
